About our Home Support Services:

MICBA Forum Italia Community Services Assistive Living Program funded by the Ministry of Health and Long-Term Care, it is based in a 60 unit, non-profit housing residence. Our program offers essential supportive services to seniors 65+ and based on assessment such services include: personal care services, basic housekeeping, social and recreational programs, in-house transportation   services,  case  coordination,  visiting  clinician,  foot  care  specialist ,  and  mobile

hairdresser, etc. The program is in operation 24/7/365 and is delivered by qualified Personal Support Workers free of charge to clients based on individual assessment. Our goal is to support seniors to live in their home while providing needed supportive services to assist them to live independently with dignity and choice.

Our most recent project has been our extension of a Supports for Daily Living Program which took place in July 2009 into  our adjacent  apartment  tower at 195 Forum Drive,
located on our property. We have provided care to clients who have the greatest care needs and require 24/7/365 supports. Our focus is to keep them living independently at home.

Forum Italia’s ethnic diversity encourages many exciting opportunities to share and grow in knowledge and experience. We serve clients who are from a wide range of cultural backgrounds. Our staff can speak English, French, Italian, Polish and Phillipino and are always culturally sensitive to the needs of all our clients.
   
The Assisted Living Program Offers:
   
The service includes assistance with activities of daily living such as: medication supervision, vital sign monitoring, assistance with basic hygiene, bathing, emergency response and daily safety checks.

• Assistance with basic activities of daily living such as:
• personal hygiene
• dressing/undressing
• medication reminders/assistance
• bathing
• basic meal preparation
• emergency response
• basic skin and wound care
• attendant services

Transportation Services

• essential transportation services by appointment to medical, grocery shopping, banking and other social related appointments within Mississauga catchment area, via our company minivan
• service is available Monday-Friday from 9:00am-1:00pm

Security Features

• personal security checks by staff
• 2-way voice communication equipment in each unit recommended to purchase by client, can be connected to staff on duty
• Superintendent, Program Director and Personal Support Workers on duty 24 hours a day in the event of an emergency

Allied Health Services

• Bi-weekly visits by Italian speaking doctor
• Monthly visits by Chiropodist
• Monthly visit by mobile hairdresser
• Supportive listening and counseling
• Referral to community/nursing services
• Working relationship with local pharmacy
• Falls Prevention Program, delivered by a qualified physiotherapist 3 x week
• Monthly Assistive Devices Repair Clinic (i.e. fixing walkers, wheelchairs, etc)

Educational Services

• Monthly information sessions on medical health topics such as: hearing screening, blood pressure and cholesterol testing throughout the year
• Annual Community Volunteer Income Tax Program

Our Supports for Daily Living (SDL) program offers:

Supports for Daily Living (SDL) enables seniors to continue living independently in their own homes by providing daily, pre-scheduled in-home support and attendant care services at a time in their lives when intermittent support over a 24-hour period, seven days a week, is needed to meet their needs.

What makes our services unique:

• We provide intermittent care throughout a 24-hour period, seven days a week
• Periodic visits throughout the day can last for as little as 15 minutes/visit
• We offer a 24-hour urgent response system that allows clients to immediately connect with SDL services in the event of an unplanned situation that falls outside the pre-scheduled visits by SDL staff
• SDL services are offered in client’s homes
• Our services can be supplemented with community support and professional services provided through the CCAC

Hours of Service:

• An average of 1.5 hours per day, 7 days a week, 365 days a year
• Scheduled in pre-determined time slot(s) at any time throughout the day, based on client need and preference (e.g. split over several visits within a 24 hour period)

24-Hour Urgent Response:

• Clients can reach us via an electronic urgent response system at their own expense or by phone 24 hours a day with urgent requests for supports that fall outside the pre-scheduled client visits by SDL staff.

SDL Services:

• Personal Support Services
• Washing/bathing
• Mouth care
• Routine hand and foot care
• Hair care
• Preventative skin care (not wound care)
• Transferring/positioning/turning
• Dressing/undressing
• Assistance with eating
• Toileting
• Range of motion exercises
• Escorting to medical appointments
• Medication reminders and/or assistance (pre-measured)

Homemaking Services

• Light meal support
• Light dusting, sweeping, vacuuming
• Mopping floors
• Washing dishes/countertops
• Bed making and laundry
• Clean and disinfecting bathrooms
• Essential paperwork

Daily Safety and Reassurance Checks

• In person or by phone

   
Who is Eligible?
   
To be eligible for Supports for Daily Living services, the applicant must be a person who:

• Is a seniors over the age of 65 years who requires daily person support services
• Demonstrates a need for daily access to personal support services throughout a 24 hour period (e.g. may have a history of falls, may require toileting assistance at night, assistance transferring)
• Is able to direct his/her own care or have a substitute decision-maker or a live-in caregiver to direct care
• Is bale to communicate his/her needs (with or without aides)
• Is medically stable (medical/professional needs can be met by CCAC, family physician or other community providers)
• Poses no risk to themselves or others
   
The RAI-CHA evidence-based assessment tool is used to help health care professionals determine eligibility for the service. The candidate must have a RAI CHA or RAI HC score of a MAPLe 3, 4 or 5. The RAI HC MAPLe score can be used if completed at time of referral, otherwise a RAI CHA is required.
