Our Supports for Daily Living (SDL) program offers:
Supports for Daily Living (SDL) enables seniors to continue living independently in their own homes by providing daily, pre-scheduled in-home support and attendant care services at a time in their lives when intermittent support over a 24-hour period, seven days a week, is needed to meet their needs.
What makes our services unique:
We provide intermittent care throughout a 24-hour period, seven days a week
Periodic visits throughout the day can last for as little as 15 minutes/visit
We offer a 24-hour urgent response system that allows clients to immediately connect with SDL services in the event of an unplanned situation that falls outside the pre-scheduled visits by SDL staff
SDL services are offered in clients homes
Our services can be supplemented with community support and professional services provided through the CCAC
Hours of Service:
An average of 1.5 hours per day, 7 days a week, 365 days a year
Scheduled in pre-determined time slot(s) at any time throughout the day, based on client need and preference (e.g. split over several visits within a 24 hour period)
